The Sacrament of Baptism requires at least one sponsor (or godparent) for each baptized person. 

Each sponsor must meet the following requirements:

(1) Be at least 16 years old;
(2) have received Baptism, First Communion, and Confirmation;
(3) live a life in harmony with the Church;
(4) regularly attend Mass and support the Parish
 
Have your sponsor complete a sponsor form below if they are a registered parishioner at St Teresa of Avila. 

If they belong to another parish, they should call the Parish they belong to get a sponsor certificate validating their position as a godparent for St Teresa's.
